MIPS CATEGORIES

For practices participating in the 2019 GPAC Medicare Shared Savings Program, you will be scored based on the MIPS APM scoring as indicated below:

 

Quality- 50%  

Eligible clinicians in the GPAC ACO get a quality performance score based on the CMS Web Interface and CAHPS for ACO quality measures that are reported by the ACO.

Promoting  Interoperability-30% 

Eligible clinicians in the GPAC ACO report at the group or individual level. 

Data is aggregated and weighted to get a single GPAC ACO score that applies to all eligible clinicians in the GPAC ACO.

Improvement Activities-20%

Eligible clinicians in the ACO get full credit based on ACO participation. 

No additional reporting is necessary.

N/A under the APM Scoring Standard.

 

Low Volume Threshold (LVT) 

Determined at the ACO level. This means that even if clinicians or groups are at or below the low volume threshold of $90,000 in allowed charges for covered professional services under the Medicare Physician Fee Schedule (PFS), furnishing 200 or fewer covered professional services to Medicare Part B beneficiaries, or furnishing covered professional services to 200 or fewer Medicare Part B beneficiaries, if they bill through the TIN of an ACO participant or if the group is an ACO participant, they will be subject to MIPS if the ACO exceeds the low volume threshold. It is rare that an ACO does not exceed the low volume threshold.

IMPORTANT INFORMATION TO NOTE

 

Public Health and Clinical Data Exchange EXCLUSIONS

Each of the five measures has their own exclusions; please refer to the Measure Specifications Sheets for the exact exclusion criteria for each measure. Generally speaking, the exclusions are based on the following criteria: 

1. Does not diagnose or directly treat any disease or condition associated with a agency/registry in their jurisdiction during the performance period. 

2. Operates in a jurisdiction for which no agency/registry is capable of accepting electronic registry transactions in the specific 

standards required to meet the CEHRT definition at the start of the performance period. 

3. Operates in a jurisdiction where no agency/registry for which the MIPS eligible clinician is eligible has declared readiness to receive electronic registry transactions as of 6 months prior to the start of the performance period. 

 

Low Volume Threshold (LVT) 

Determined at the ACO level. Attention all MSSP Members,

Please review the below information and tools from CMS on Medicare Revalidation.

 

The Shared Savings Program requires all ACO participants to be enrolled in Medicare (42 CFR § 425.20). Section 6401 (a) of the Affordable Care Act established a requirement for all enrolled ACO providers/suppliers to revalidate their Medicare enrollment information under new enrollment screening requirements. In order to streamline the revalidation process and reduce the ACO providers/suppliers burden, CMS implemented several revalidation processing improvements. We believe these tools are helpful for reminding your ACO providers/suppliers to revalidate timely and maintain their Medicare enrollment in order to minimize any adverse impact to their ACO participant status. The most efficient way for a provider or supplier to submit revalidation information is by using the Internet-based Provider Enrollment Chain and Ownership System (PECOS) on the CMS website https://pecos.cms.hhs.gov/pecos/login.do#headingLv1 . PECOS allows you to review information currently on file, update, and submit a revalidation via the Internet.

 

It is important for providers and suppliers to keep all of their contact information in PECOS up-to-date so the Medicare Administrative Contractor (MAC) can contact them as needed. A listing of all currently enrolled providers/suppliers is available by using the Medicare Revalidation List https://data.cms.gov/revalidation . You can search for a provider/supplier by last name, first name, organization name, or by National Provider Identifier (NPI),or download a list of revalidation due dates. Those on the list due for revalidation will display a revalidation due date, all other providers/suppliers not up for revalidation will display a “TBD” (to be determined) in the due date field.

 

The revalidation due date will be posted up to six months in advance of the revalidation due date to provide sufficient notice and time for the ACO provider/supplier to comply. The revalidation due date list is refreshed once every two months on or around the first of the month and two months of due dates are appended at that time. 2016 MSSP Participants ONLY

 

When GPAC satisfactorily reports the GPRO Quality Measures and Administers the CAHPS Survey for 2016, ALL providers that bill under an approved 2016 GPAC participant TIN will: 

AUTOMATICALLY-Avoid the 2018 PQRS negative payment adjustment (-2.0%)

OPTIONAL-Satisfy the CQM component of the Medicare EHR Incentive Program(during the MU Attestation you will notify CMS if you chose to have the ACO quality measures also satisfy your MU CQM's)

NOTE-EP's will still be required to report the other MU objectives through the MU registration and Attestation system.

AUTOMATICALLY-The 2018 Value Modifier adjustment will be determined by the data submitted by GPAC and will vary by the composition and size of the participant TIN.

 Medicare Shared Savings Program Participants (MSSP)-

The GPAC Medicare Shared Savings program (MSSP) requires 34 quality measures to be reported for 2016.  As a participant of the GPAC MSSP you need to be aware of all the measures and be advised on how to meet the 18 quality measures that will be collected from your EMR for data submission.  Please note that this data submission will satisfy your PQRS requirement for 2016, fulfill your Value Modifier and can also satisfy your the CQM portion of the EHR Meaningful Use attestation.

The GPAC United Healthcare Shared Savings program requires 12 quality measures to be reported for 2016. As a participant of the GPAC UHC shared savings program you need to be aware of all the measures and be advised on how to meet the 10 quality measures that will be collected from your EMR for data submission.
